From my recollection basically the ESMTP fixup in the PIXOS 6.x and earlier has always caused problems of sorts with ESMTP. Although not completely relevant Microsoft has a kb on potential pix smtp fixup issues you may see. http://support.microsoft.com/kb/320027

I have even seen issues in the modern version of fixups in the pix/asa inspect command. I generally leave both disabled because of the inconsistencies while having it on.
