Definitely the problem... Outlook was set up with a 1min timeout.

mx1 spamd[40059]: clean message (-103.1/5.0) for mailnull:26 in 81.1
seconds, 8782242 bytes

Given this was taking longer than 60 seconds, outlook was resending!

At least attachment problem solved!
