I had a number of problems with that at one of the locations I oversee.
They were totally intolerant of any changes to the subject line and the
fuzzy area of 4 though to 6 of spam assassin scores were either jokes
(that they really wanted) scoring 6 or clever spam getting 4.

What I did was to set anything over 16 to be rejected with a pleasant
message explaining why (These were all spam anyway so far). Anything
from four upwards I froze using an ACL. We accept only valid email
addresses which bounces a lot of spam and following the first week where
we caught 1000+ spam we are down to about 30 messages frozen a day.

I set up a web page for one of the secretaries to check the messages and
release or reject the messages. We receive just less than your volume
at that location and everyone is much happier.

We can of course whitelist all of the companies we regularly deal with.
