Almost certainly caused by a quote error. Did you try

*@*     "${lookup_mysql{MYSQL_REWRITE}{$value}fail}"       T

If so, then perhaps there are quotes inside MYSQL_REWRITE? Remember, this
is just a macro, so it gets substituted before the line is interpreted.
