I've only ever seen this when I've tried to get clever by editing the
spool (-H) file directly. Could you post the relevant spool file here,
all of the -H file, only the first line of the -D file is relevant
AFAIK, and someone may spot the problem.
On a similar subject (well, very vaguely similar, it's at least to do with
spool files)
$ ls -l /var/spool/exim/msglog
total 3
-r-------- 1 exim mail 32 Feb 6 19:48 109DhV-0009l6-00-37641-0
-rw------- 1 exim mail 182 Feb 11 13:03 10AtBC-000Akf-00
-rw------- 1 exim mail 172 Feb 11 13:03 10AtIu-000Akw-00
Shouldn't that first file have been deleted when message
109DhV-0009l6-00 was finished with? (fwiw, the file contains some output
from a pipe command.) If this is an Exim problem, I imagine these
leftover files could accumulate quite a bit on a busy system.
