I have encountered the same problem, upgrading from 4.85 and compiling 4.86.2
from source. In my case the system-filter wasn't in use, however. All messages
from our environment are forwarded through Google's SMTP Relay service, and
nearly all of them were in the state that Steve describes: messages had been
delivered (though reporting the "smtp transport process returned non-zero
status 0x000b: terminated by signal 11") but the queue still reported them as
present on the server. Subsequent delivery attempts reported "Spool file XXXX
not found". Manual delivery attempts with 'exim -M <ID>' failed with the same
error.

Reverting to 4.85 and reattempting the manual delivery corrected the problem.
