Thanks for the info. It shows me that it is in fact updating (appending)
the file. It must be Dovecot POP3 that isn't appending delete entries to
"free up" the space. I'm looking in that direction now.

You can debug this problem by sendind a mail from exim command line like
this:
exim -d+all mail_with_maildirsize
I got this concerning maildirsize in my server:
