IMO, ideally it would default to requiring HELO/EHLO, but with a switch to turn
that requirement off. However, that's not a backward compatible change so can't
be put in a minor version update eg 4.69-4.70. It would probably belong in a v5
if one is ever created.
Perhaps until then, the best alternative is to add a small config snippet to
the default config.
