mmm ... i'm using it for years and never seen as virus pass through it.
Anyway i must say that we catch almost all viruses by recognizing helo
patterns and that very little reach clamav.

then the solution could be to do the virus scan at the transport ...
but i also remember that there was a recent topic talking about a way to
know at smtp time where each address will be routed (by setting
address_data) and then accepting only one recipient per message but
i do not remember is there was a practical solution, maybe you can find
the answer in the archives.
