No problems here - been using this method for over a year now, never a
double Subject: header.

        driver                  = smtp
        multi_domain            = no
        size_addition           = -1
        retry_use_local_part    = yes
        headers_remove          = Subject
        headers_add             = X-Delivered-To:
<${local_part}@${domain}>\n\
                                  Subject: ${if and {{first_delivery} {eq
{$header_X-Spam-Flag:}{YES}}} {*SPAM* $bheader_Subject:}{$bheader_Subject:}}


Then again, note the use of "first_delivery".
