Gmail supports authenticated SMTP, so you can use them as your
smarthost. should be more dependable than most ISPs, anyway.

this is a FAQ on this list :-) I don't think there has been a good
answer for it. perhaps a callout can be abused? the best answer is
probably "don't", though.
