It can occur if you let users get involved in address routeing. See for
example the documentation for check_ancestor. A user may quite
legitimately put one of their own addresses in a .forward file in order to
receive a copy of email locally as well as at another address, and the
user may also quite legitimately not distinguish between an alias and a
"real" address.
