What I got now is with a different erro msg when sending email from
Outlook from external. The error is "Relay not permitted". But sending
email locally within a relay-permitted domain is fine. It is because
when sending email locally, Exim will use lmtp local transport. But when
sending from external, the authentication will be dne with following
Auth rules:
fixed_login:
driver = plaintext
public_name = LOGIN
server_set_id = $1

sasl_cram_md5:
        driver = cyrus_sasl
        public_name = CRAM-MD5
        server_set_id = $1


sasl_plain:
        driver = cyrus_sasl
        public_name = PLAIN
        server_set_id = $1
