Have you updated your db/dbm libraries recently?
Try deleting all the hint files - probably in /var/spool/exim/db
directory.

If your db/dbm interface has been changed incompatibly occaisionally you
can find crashes when exim tries to open the data files. Hint files are
transitory so can be deleted with little effect (OK, its less efficient
until hint data has been built up).
