Add local_part_prefix or local_part_suffix to your localuser router.

Also have a look at the local_part_prefix_optional and
local_part_suffix_optional settings.

The default settings according to
http://www.exim.org/exim-html-4.30/doc/html/spec_7.html#SECT7.3
should be

localuser:
    driver = accept
    check_local_user
    transport = local_delivery


Add the above options here.

Test with exim -bt <plussed+address@???>
and exim -bv <plussed+address@???>
