Yes, as exim will never cleans the cache by itself.

From dbstuff.h

/* The contents of the "misc" database are a mixture of different kinds
of record, as defined below. The keys used for a specific type all start
with a given string such as "etrn-" or "host-serialize-". */

Usually it shouldn't be necessary to run exim_tidydb on it, as exim
deletes the records by itself. As Andreas already suggestes, you can
safely delete this file on boot. If you still want to use exim_tidydb,
then I would suggest a small -t setting (1h or so).
