I interpret unqualified "fails" to mean a permanent failure. A temporary
error causes defer. There is a WishList item about this:
(226) 23-Dec-03 S A way of treating DEFER as fail in dnsdb lookups
(i.e. the dnsdb lookup failed, so accept the message)
