I don't believe every mailserver on earth and other places should run a
virus scanner. My personal server does not scan any email. There's
absolutely no reason to. There's no windows machines on my network and I
don't accept email for unknown localparts. Infact, dew to swen my primary
MX is not reachable by the internet (only by the secondary and that's
because of limited bandwidth).

Simply, if you don't cater to windows and/or macos, you don't really need
virus scanning.
