[Exim] exim rejecting mails from sites that were ok.

We have recently moved from exim 2.x to exim 4.12. We are getting lot
of mail reject.
Our ACL is

One such E-mail address is 62.254.211.4

another one is 193.130.228.132.

Previously they could send us e-mail - but not from yesterday.

Any help would be appreciated.
