That both your successful attempt and your failed attempt show connection
refused to various hotmail MX records I think indicates that hotmail is
having load problems at the time you did these tests. The have 14 MX records
and each of those listed servers is actually a load balanced IP for a much
larger number of real mail servers. It is not uncommon for them to fail
connections on significant number of their servers for an extended period
of time.

I think, however, if you run a few instances of 'exim -v -Rf @hotmail.com'
in quick succession from the server where your mail has not yet been
delivered, it should probably deliver after a few tries.
