The header is added by an MTA on another server. The message has the
header (or both headers) for the entire time exim sees the message.

OK, that was it. I was using "is". I changed the filter rule to "matches"
and it started working again. I knew there were two instances of
the header, I just couldn't find the definition in the book for
behaviour in this situation. I see now though that it shows up in -d99 as
"Subject = " and "Pattern = ".
