That pointed me in the right direction. I had permissions issues with my
forward files as well. (They have to be 750?) but once I added
no_check_local_user into the mix, at lest the log file started telling me
there was a problem.

user = LOOKUP_ACCOUNT
group = LOOKUP_ACCOUNT
The capitalised things are macros, but they DTRT. I think the one you are
missing may be "check_local_user".
