With that log set I can see how you get 2 deliveries but not 3...
You are getting one delivery to the mailbox directly:-
2000-10-30 12:29:04 13qD7g-000124-00 => usera
<usera@...> D=localuser T=local_delivery
And two to the procmail pipe (so why are there 2 to the procmail pipe -
time to run a exim -d99 -bt on that address and see what you get
