People who use inetd for smtp probably don't have that much traffic that
CPU time is a problem, otherwise they would not use inetd. ;)

You are right about cron and MUAs, but on mail hubs, you can neglect
those. An exceptions might be queue runners started by cron, but I
don't see their use in systems with high traffic. Exim cares about not
starting more queue runners than specified and cron might overload the
system if the queue runners from the previous cron job are still at work.

The thing about orphaned children is a real problem, though.
