Difficult to say.

No, it was a server problem, but it was with Exim 3.00, in which changes
have been made since 2.12. There haven't before been problems like this
with 2.12, as far as I know. Is there a specific message that provokes
this behaviour? Can you repeat the problem, or is it intermittent? If I
could find a message that provoked this, I might be able to track it
down. The only common feature among the reports is Linux, incidentally.

The first step at debugging is to start the server daemon in debugging
mode, e.g.

exim -d9 -bd

and collect the stderr output, which will be quite large. If you can do
that from a failing case, it might throw some light on the problem.
