Indeed it doesn't. It doesn't know about X- headers. They can be
anything you like. It rewrites only those standard headers which it
knows contain lists of addresses in a defined syntax. For example, it
will not rewrite an address that appears in a Subject: header.

No RFC reason. RFCs are concerned with what is on the wire between
hosts. What you do inside your host is up to you. It would be a pain to
specify, I think.

Only way to rewrite them is either to set up a transport filter, or do
something cunning with headers_add and headers_remove.
