My guess is that:
(1) You have set the "sender_verify_fixup" option set.
(2) A message came in with a return path of gyang@???.
(3) The DNS lookup of worldnet.att.net timed out.
(4) Exim tried a fixup by looking up the address in the From: line.
(5) This time the DNS lookup of worldnet.att.net did not time out.
(6) Exim isn't clever enough to notice that the two addresses are the same.
