The MIME parameter handling currently knows about only a few parameter names
(particularly: filename & name). Unknown parameters just get skipped. It
would
be good to provide some sort of visibility of both name & value of all of them
so
that custom handling of, say, URLs could be coded in ACL.

Doing RFC2231 handling for continued values seems reasonable.
RFC2231 charset/language support would be harder.

A space-sep list of name=value would match the ${extract } expansion item.
