Is this file even referenced from exim.conf?
As Jeremy writes, you're using a Debian specific configuration
mechanism. Debian has its own way of configuring exim, on top of
exim's native mechanism. The main bug in your setup is very likely in
the Debian layer.
The existence of these 2 separate layers is a constant source of
frustration for people who come asking here what to them are perfectly
sensible questions. But the upstream exim project can hardly be
blamed for that.
