Always start with the mainlog. Find a delivery try for
the message-id. If you can't find one - are you
actually doing regular queue runs?
If that doesn't give a clue then fire a try manually,
with debug enabled:
exim -d+all -M THE_MESSAGE_ID 2>&1 | tee debuglog
I suggest capturing the debug output because it's
a lot of complicated info, and will take you a while
to interpret.
