If the changes are to spamassassin rules / configuration, there is no reason to restart exim at all.

If the rules are in files and are applying to the global configuration, you typically would need to restart spamd (the spamassassin daemon).

That makes sense. Typically a cron job updates spamassassin rules and compiles them and restarts spamassassin.
