Which version of ClamAV are you running? Recent versions load the
definitions into RAM alongside the current set, and then do an atomic
switch. This means there is almost no pause while defs are reloaded.

On the Exim side, you could add a long timeout to the "malware =" line but
be warned, you can end up with your listening daemon refusing connections
while it waits, as it could hit your inbound connection limit.
