I'd remove the short form of --man (-m), as I believe, *this* cannot be in wide
use in scripts. For -M|--related I can imagine scripts using it.
Keeping case-insensitive option matching is a good move, I think. Not because I
like it, but you are right, we do not know, if there are scripts around,
relying on options being case insensitive.
