2 things you can try
1) /usr/sbin/exim4 -bd -d+all
and check the logs to see what happens and whether local_scan gets run.
2) strace -f -e trace=file -s 256 /usr/sbin/exim4 -bd -d+all
check the strace output to see whether tries to load local_scan as it
should
