The situation changes with TLS1.3 - OCSP stapling is carried in a different way
which allows intermediate-cert status up front, and the status_request_v2 is
not needed (under 1.3) and deprecated.
We may still need coding changes for handling such (if the library doesn't
Just Do It in some useful fashion), and for observability. We will certainly
want cases in the testsuite.
