In 4.82, the remove_header ACL modifier was implemented, as per bug 198.
However, the docs do not reflect that, which makes it hard to figure out why
the feature doesn't work with earlier Exim versions.

The remove_headers entry here should have a note that the feature was
introduced with 4.82.
