Hello,
this seems to be a report about the Debian exim configuration, not a upstream
issue. Please do not send these type of questions to the upstream tracker.
There is a special mailing list for dealing with Debian specific exim questions

Regarding your report:
exim4.conf.localmacros allows setting macros, not main configuration settings.
And yes, there is macro for overriding keep_environment. See
grep -2 keep_environment /etc/exim4/exim4.conf.template
