Assuming you've already checked drive health.

it could be directory bloat. if you've had a queue explosion the spool directories may be large but empty.

stop exim and do this

# cd /var/spool/exim/
# cp -al input input2
# rm -r input
# mv input2 input

then start exim again.

that will re-build the spool directories to a smaller size.
