First, if you're running Exim 4.76 - update it.
Then, assuming the problem still exists: restart your daemon with a
commandline
debug option, collecting output. Feed it a test mail. Examine the debug
output,
which shows the processing flow for the message. Compare with your config and
work out where it differs from what you expected.
