Monitoring now in place. All I need to do is get the user who uses the
user@??? to route their email back to that email address
again (haven't seen the bug since 2016-02-08, after which they got fed up of
losing emails and routed their email elsewhere).

I think that the regular "421 ... Too many connections from same IP" response
from Plus.net's mail servers is helping to expose this problem, by causing
subsequent messages to be deferred. I've not done anything about this so far
but once I've finished collecting data for this bug report, I'm going to use
the serialize_hosts setting for plus.net MX servers to prevent this from
occurring.
