If the clamd log indicates it wasn't handed the EICAR
for scanning:

Do that again, but first (in another window) shutdown your
exim daemon and run it with the option "-d-all+acl+expand"
added to the normal commandline (check that first with ps).

This will output voluminous debug (you might want to capture
to a file) from which you should be able to see the flow
through the ACL processing, and work out why you're not
scanning that message.
