How did you get the logs? The %22 and "mailto:…" may be the cause of
your problems, or they're added by the software you used to copy/paste
the logs.

If(!) the logs are as they are, the problem is the Sender address, used
as

    "mailto:…"

Talking about Sender verification, that is, verifcation of "mailto:myaddress@mydomain.com%22@???".
Exim is responsible for the sender domain, thus it tells you more about
the cause (the unwanted characters).
