Yea. I've been struggling with this for years, actually. The documentation
indeed is misleading.

I'm trying to pull myself together and get back on top of exim issues. :-)

The reason why I wouldn't want to remove/add headers in ACL is due to my
preference to perform modifications 1) only to inwardly-delivered copies
(excluding copies forwarded back out) and 2) all in one place, respecting
per-user preferences. My configuration is a little complex: I do a lot of
processing and decision making in exim, e.g. add a header like X-Spam-Folder:
Yes to be processed by sieve, rather than let sieve do an integer comparison on
the spam score. This makes it easier for me to set up a rather simple user
configuration interface with an SQL backend.
In this setup adding spam_score is rather for informational/troubleshooting
purposes.
