Thank you for this very comprehensive bug report. I have reproduced the
problem. It is associated with the use of (*ACCEPT) within capturing
parentheses, but I have not yet had time to work out the details. As well as
showing up a bug in PCRE, this also shows a deficiency in pcretest, which
should probably be putting junk in the ovector, as you did, to catch issues
like this.

I will try to post a patch sometime soon, but it may not be until the weekend
if this takes some time to sort out.
