I'm not completly sure, but I'd say, Exim takes care a lot about locking
issues. It should be NFS proof, but probably depends on the NFS
implementation you're using.

Creating the -H and -D files in an NFS spool should be possible
without problems. There is a 'localhost_number' that helps creating
unique file names in such shared spool/input directory.

During spool runs locking issues could occur, during the delivery a
journal file (-J) tracks the progress. You should check the source if
there are some hints about NFS and how this file is created and
protected against races between your servers.

Locking issues on the hint files may drain your performance, but you're
not asking about spool/db :)
