I differ in what I would expect after first email delivery:

To me the master concept is that Exim should only create maildir folders inside
valid top level maildirs.
However, the opposite point is that for the first inbound email to a new
mailbox, your transport specifically set the root maildir directory to a path
you know is actually a folder, but nothing indicates to Exim what actually is
the top level maildir. I don't know if this is solvable without one of the
following paths:
1) another maildir_* option.
2) assume one subdirectory down is root maildir and create {tmp,new,cur}/.
3) make it fail with undeliverable.
I don't know that I like any of those. Any other suggestions?
