I'd do it in a router on debian.

With debian's split configuration option you can just drop the router
in as a file in /etc/exim4/conf.d/router

eg: /etc/exim4/conf.d/router/50_example.nl_rewrite

if you're gone for merged config you need to edit
/etc/exim4/exim4.conf.template and insert the new router.
