I forgot to add that I did try
/usr/share/doc/exim4-base/examples/exim-gencert
which made some certs which also didn't work.
There is some mention of
/usr/share/exim4/exim4_refresh_gnutls-params
in the docs, but that doesn't exist anywhere in my debian package, and I
do not have any /var/spool/exim4/gnutls-params
