If you use the router option caseful_local_part, the unchanged (caseful)
local_part seems to be used for the transport too. (I write "seems", as
I can't find a relevant note in the spec file.

Just tested it with a micro config:

    ~~~[exim.conf]
    begin routers
            rrr:
                    driver = accept
                    caseful_local_part = true
                    transport = ttt
    begin transports
            ttt:
                    driver = pipe
                    debug_print = "Using: ${local_part}"
                    command = /bin/true
                    # optionally
                    # command = /bin/sh -c "echo '<${local_part}>' > /dev/tty"
    ~~~

# date | exim -C <micro-config> -v xxx@localhost
vs.
# date | exim -C <micro-config> -v xXx@localhost

    Best regards from Dresden/Germany
