He sent me this bit of illumination offlist. It explains the behavior:
The binary is standard debian. md5sum matches.
This is how you can get split logs:
.ifdef OUTGOING
SPOOLDIR=/var/spool/exim4
.else
SPOOLDIR=/var/spool/exim4_incoming
queue_only = true
queue_only_override = false
log_file_path = /var/log/exim4_incoming/%slog
